What an endodontist treats

Endodontics deals with the inside of the tooth — the pulp, the nerve and the root canal system. It is the specialty you meet when a tooth is painful, infected or abscessed, and it exists to keep the natural tooth in place.

That aim is worth stating plainly, because it runs against the assumption many patients arrive with. A tooth that hurts is not automatically a tooth to extract and replace with an implant. Where the root is sound, treating it is usually the better outcome — your own tooth root, kept.

It also matters for planning. An untreated infection under a tooth that is about to be crowned, or next to a site where an implant is planned, has to be resolved first or it compromises the work built on top of it.

Do I need a root canal or an implant?+
Where the root of the tooth is sound, root canal treatment keeps your own tooth and is usually the better outcome. An implant is the answer when the tooth cannot be saved. Which applies to you is a clinical judgement made from an examination and x-ray, and it is made before any extraction.
Is root canal treatment painful?+
The procedure is carried out under local anaesthetic. In most cases the pain that brought the patient in is caused by the inflamed or infected pulp itself, and treating it is what relieves it.
My tooth hurts but my dentist cannot find anything. What now?+
Referred dental pain is common — the tooth that hurts is often not the tooth with the problem, and pain from an upper molar can be felt in the lower jaw or vice versa. Locating the true source is part of endodontic diagnosis.
Can a previously root-treated tooth be treated again?+
Frequently, yes. Retreatment reopens the tooth, removes the previous filling material, re-disinfects the canal system and seals it again. It is often possible where a tooth has been declared hopeless elsewhere.
Does an infection have to be treated before a crown or implant?+
Yes. Building a crown onto an infected tooth, or placing an implant next to an untreated infection, puts the new work at risk. Infection is resolved first, which is why endodontic assessment forms part of planning a larger case.
